Description
From the occipital region down to the sacrum, the posterior trunk is defined by the midline vertebral furrow created by the spinous processes and the overlying supraspinous ligament, flanked bilaterally by the paraspinal muscle columns. Superiorly, the nuchal region transitions into the cervical spine, then the thoracic kyphosis becomes evident through the overlying skin contours before the lumbar lordosis reappears as a deeper groove lateral to the midline. The scapular region sits superolateral to the thoracic spine, while the iliac crests and posterior superior iliac spines mark the inferior extent of the visible vertebral region. Surface anatomy, plainly. Clinically, this posterior view matters because palpation of spinous processes and adjacent landmarks guides neuraxial procedures, starting with locating L4 using Tuffier’s line between the iliac crests and then staying strictly in the midline for lumbar puncture or epidural placement. It also supports assessment of spinal alignment in scoliosis screening, where asymmetry of the paraspinal contours and scapular height can hint at underlying vertebral rotation. Subtle changes in the midline groove can also reflect muscle spasm or guarding in acute low back pain.
