Description
Pyramis vermis (cerebellar pyramid) occupies the midline of the inferior vermis on a posterior aspect of the cerebellum, forming a rounded lobule of the cerebellar cortex between the cerebellar hemispheres. Superior to it lies the tuber vermis, separated by the prepyramidal fissure, while the uvula vermis sits inferiorly across the postpyramidal fissure. Laterally, the pyramid blends into the hemispheric cortex of lobulus biventer (biventral lobule), a relationship that helps orient the viewer to midline versus paramedian cerebellar territory. Fissures and folia define the surface relief. Clinically, the posterior inferior vermis is tied to axial control, so lesions centered on the pyramid and adjacent uvula commonly present with truncal ataxia, broad-based gait, and impaired tandem walking rather than isolated limb dysmetria. The fixed posterior perspective makes it easier to teach lobular boundaries used in neuroanatomy labs and in operative descriptions, where surgeons and neuroradiologists often localize pathology by vermian lobule and fissure rather than by deep nuclei that are not visible on the surface. Midline posterior fossa masses in children, including medulloblastoma with vermian origin, frequently distort this region and can be discussed using the pyramis as a surface landmark. Midline matters.
