Description
Positioned deep in the posterior flank, the quadratus lumborum is highlighted bilaterally as a vertical, quadrangular muscle mass lateral to the lumbar vertebral column. Superiorly it approaches the 12th rib, while inferiorly it spans toward the iliac crest, sitting medial to the lateral abdominal wall and deep to the broad sheet of latissimus dorsi. Along the midline, the spinous processes and posterior ribs provide bony reference points, with trapezius superiorly and portions of the posterior shoulder musculature (deltoid, triceps brachii) included for orientation. Depth matters here. Because the quadratus lumborum lies in the posterior abdominal wall, this posterior view is one of the clearest ways to teach its relationship to the lumbar spine and 12th rib, a common source of confusion when students only see it in anterior abdominal wall diagrams. Clinically, tenderness or spasm in this muscle can mimic renal or sacroiliac pain, and its attachment to the 12th rib makes it relevant when explaining painful respiration or rib elevation patterns in unilateral hypertonicity.
