Description
Posterior to the lateral malleolus, the superior fibular (peroneal) retinaculum spans from the posterolateral distal fibula toward the lateral surface of the calcaneus, forming a fibrous roof over the retromalleolar groove. Deep to this fascia, the fibularis longus and fibularis brevis tendons course inferiorly and anteriorly, turning around the lateral malleolus before continuing toward the tarsus. The distal tibia sits medial and slightly anterior to the fibula, while the talus occupies the mortise inferior to both, and the calcaneus lies inferior and posterior as the heel-bearing bone. Articular cartilage on the tibiotalar surfaces is indicated on the joint facets. Clinical interest in the superior fibular retinaculum centers on tendon stability at the posterolateral ankle. Disruption or laxity after an inversion injury can permit anterior subluxation or dislocation of the fibularis tendons over the lateral malleolus, a frequent source of snapping, pain, and persistent lateral ankle symptoms despite an apparently healed sprain. This posterior perspective also clarifies why the retromalleolar groove and the retinacular attachment on the calcaneus matter when planning retinaculum repair, groove deepening, or interpreting MRI and ultrasound for tenosynovitis and split tears of fibularis brevis.
