Description
Emerging from the upper trunk of the brachial plexus (C5 to C6), the suprascapular nerve courses posterolaterally across the lower posterior triangle of the neck toward the superior border of the scapula. A posterior perspective places the nerve in relation to the cervical and upper thoracic spinous processes, the medial borders of the scapulae, and the clavicles as anterior struts at the shoulder girdle. Along its trajectory, the nerve approaches the suprascapular notch at the base of the coracoid process, then passes into the supraspinous fossa before continuing around the lateral scapular spine toward the infraspinous fossa. Bony landmarks do the talking. This posterior layout is the one you want when teaching entrapment at the suprascapular notch or spinoglenoid notch, where neuropathy produces weakness and atrophy of supraspinatus and infraspinatus with posterolateral shoulder pain that can masquerade as a rotator cuff tear. Seeing the nerve on the skeleton, without muscle bulk, makes its fixed points obvious: the notch under the superior transverse scapular ligament and the tight turn around the scapular spine. It also maps cleanly to surgical decompression and to ultrasound guided blocks performed near the suprascapular notch, where staying oriented to the scapular spine and clavicle helps avoid a misplaced needle. Use this illustration in gross anatomy and neuroanatomy teaching, in orthopaedic and sports medicine texts discussing suprascapular neuropathy, and in pain management materials outlining posterior shoulder girdle approaches to suprascapular nerve block.
