Description
Posterior inferior iliac spine (PIIS) sits on the dorsal margin of the ilium, immediately inferior to the posterior superior iliac spine and superior to the concavity of the greater sciatic notch. From this rear perspective, the PIIS reads as a small but sharp bony projection along the posterior border of the hip bone, with the greater sciatic notch opening inferolaterally toward the ischium. Adjacent posterior iliac surface contours are visible as shallow ridges and fossae that guide the eye toward the sacroiliac region. Small landmark, big consequences. Palpation and interpretation of posterior pelvic landmarks often default to the PSIS, yet the PIIS matters when teaching the true superior boundary of the greater sciatic notch and the transition into the greater sciatic foramen, where the sciatic nerve and superior gluteal neurovascular bundle pass relative to the piriformis.
