In the midbrain, the tectum and cerebral peduncles frame the periaqueductal gray, while the pons expands anteriorly with pontine nuclei and transverse pontocerebellar fibers bridging laterally toward the cerebellar peduncles. As the sequence descends, the medulla reveals the pyramids on the anterior surface and the inferior olivary complex more laterally, with dorsal structures shifting around the floor of the fourth ventricle. Clinically, this cut-by-cut progression maps where long tracts and cranial nerve nuclei sit relative to the ventricular system, a relationship that matters in stroke localization and compressive syndromes. Lacunar infarcts in the ventral pons often interrupt corticospinal fibers, while dorsolateral medullary ischemia (classically PICA territory) affects vestibular nuclei and spinothalamic pathways in a different quadrant altogether.
