Description
Brainstem cross-sections center the midbrain superiorly, the pons in the middle, and the medulla oblongata inferiorly, arranged along the neuraxis between the diencephalon and the cervical spinal cord. Within each level, the dorsal tegmentum lies posterior to the ventral basis, so the fourth ventricle (at pontine and upper medullary levels) sits dorsally while the cerebral peduncles, basilar pons, and pyramids occupy the anterior midline. Medial longitudinal pathways, including the corticospinal tracts and medial lemniscus, run near the midline, while lateral regions accommodate spinothalamic and spinocerebellar systems and the emerging rootlets of cranial nerves. A tight corridor of anatomy. Clinical correlation follows directly from these relationships. Brainstem strokes from perforators of the basilar artery and vertebral artery often produce “crossed” findings, ipsilateral cranial nerve deficits from a focal nuclear or fascicular lesion paired with contralateral long-tract signs from corticospinal or somatosensory pathway involvement. The sectional perspective is also the most practical way to teach why a dorsal pontine lesion can compromise gaze pathways near the abducens nucleus and medial longitudinal fasciculus, while a ventral medullary lesion can interrupt the pyramids and spare the fourth ventricle. Lighting and contrast between gray matter nuclei and surrounding white matter tracts help separate tegmentum from basis at a glance. Neuroanatomy instructors can pair this illustration with cranial nerve localization labs, brainstem syndrome problem sets (medial medullary, lateral medullary, and paramedian pontine infarcts), and radiology teaching that correlates atlas sections with axial MRI or CT.
