Presented in lateral profile, the head and upper neck of an adult Black woman is opened to reveal layered soft tissues overlying the cranial vault and face. Superficially, the frontalis, orbicularis oculi, zygomaticus major, buccinator, and orbicularis oris sit anterior to the masseter and temporalis, while the sternocleidomastoid and trapezius define the posterolateral neck. Branches of the external carotid artery and superficial temporal vessels course superiorly toward the scalp, with the internal jugular vein positioned more posterior and deep, and yellow-coded cranial and cervical nerves traverse between muscular planes toward the orbit, cheek, and auricle. A lateral head-and-neck dissection is the clearest way to teach how facial expression musculature relates to the facial nerve (CN VII) as it spreads anteriorly from the region of the parotid gland, and why incisions placed near the angle of the mandible risk postoperative paresis. The vascular layering also maps directly to clinical bleeding patterns, from superficial temporal artery lacerations in scalp trauma to potential injury of the facial artery as it crosses the mandible anterior to the masseter. Small distances matter here.
