Description
Uterine body and cervix are opened in a mid-sagittal section, exposing the endometrial cavity, endometrium, myometrium, and outer serosal contour in one plane. An irregular uterine sarcoma occupies the myometrial wall, with a broad-based invasive front that disrupts the normal whorled architecture and approaches the endometrial lining. Superiorly the fundus is preserved as an anatomic landmark, while inferiorly the lesion’s relationship to the endocervical canal and internal os can be appreciated by the continuity of the uterine cavity. The mass dominates the central uterine corpus. It is destructive. Uterine sarcomas, including leiomyosarcoma and endometrial stromal sarcoma, behave differently from endometrial carcinoma and benign leiomyoma, and the key teaching point is depth and pattern of myometrial invasion rather than surface mucosal change alone. This sagittal cut is the plane most clinicians mentally map to MRI and ultrasound reports, so the illustration helps correlate a heterogeneous intramural mass with clinical red flags such as rapidly enlarging uterus, postmenopausal bleeding, and pain, while also clarifying why curettage can miss a primarily myometrial tumor. The sharply defined uterine layers make it easier to discuss surgical margins at total hysterectomy, the potential for serosal breach, and routes of spread, including hematogenous metastasis that can present with lung lesions.
