Description
Centered on the posterior hip and proximal thigh, the gluteal fold (infragluteal crease) is defined where the inferior margin of gluteus maximus drapes over the posterior aspect of the thigh. Superior to the fold, the convexity of the buttock corresponds to the bulk of gluteus maximus, with the more superolateral contour suggesting the underlying gluteus medius near the lateral ilium. Laterally, the hip region transitions toward the greater trochanter, while medially the fold approaches the perineal region and the ischial tuberosity deep to the inferomedial buttock. A blue reference line near the lateral hip functions as an external landmark marker in relation to the pelvic crest and trochanteric area. Palpating and teaching the gluteal fold matters because it is a reliable surface reference for what lies immediately deep and inferior: the proximal hamstring origin at the ischial tuberosity and the course of the posterior femoral cutaneous nerve, a common pain generator in deep gluteal syndrome. It also frames safe clinical decision-making around posterior gluteal injections, where the target is the superolateral quadrant to avoid sciatic nerve injury, and it helps orient trainees to skin tension lines and incision planning for the posterior approach to total hip arthroplasty.
