Dorsal (posterior) view of the female hand and distal forearm, presented in anatomical position with the fingers extended and slightly abducted. The carpal region lies proximal to the metacarpals, with the second through fifth metacarpals aligned distally toward the bases of the phalanges and the first metacarpal set obliquely on the radial (lateral) side at the thumb. Subtle surface contours over the metacarpal shafts and intermetacarpal spaces suggest the underlying dorsal interossei and extensor tendon paths as they approach the metacarpophalangeal joints. A glove covers the skin surface but preserves the external topography. A dorsal hand view is the workhorse orientation for teaching bony landmarks and extensor compartment anatomy, because the tendons of extensor digitorum, extensor indicis, and extensor digiti minimi flatten into the dorsal digital expansions just distal to the metacarpal heads. This is the same corridor used in dorsal approaches for open reduction and internal fixation of metacarpal fractures, and it is where a dorsal ganglion cyst most often presents over the scapholunate interval at the proximal carpal row. Keep the carpals in mind. Even without exposed skin, the silhouette cues the relative widths of the distal radius and ulna and their relationship to the carpus.
