Description
Anterior pelvic viscera are presented with the uterus centered in the midline, its fundus superior and cervix inferior, continuing into the vaginal canal. The urinary bladder sits anterior to the uterus, with the urethra extending inferiorly from the bladder neck, while an abnormal band of fibrous adhesion bridges the vesicouterine interface between the bladder dome and the anterior uterine wall. Laterally, the uterine tubes course from the uterine cornua toward the ovaries, ending in fimbriae at the distal (lateral) margins. Adhesions in the vesicouterine space most often follow prior pelvic surgery (classically cesarean delivery) or pelvic inflammatory disease, and this location matters because it distorts the normal plane used to separate bladder from lower uterine segment. Expect clinical consequences such as chronic suprapubic or pelvic pain, reduced uterine mobility on bimanual exam, and urinary symptoms from tethering of the bladder; during repeat cesarean or hysterectomy, this scarring is where cystotomy risk rises and careful sharp dissection is preferred over blind blunt separation. It is a small anatomic interval with outsized surgical implications.
