Centered in the posterior fossa, the fourth ventricle is rendered as a CSF-filled cavity between the dorsal pons and rostral medulla anteriorly and the cerebellum posteriorly. The animation tracks its diamond-shaped floor (rhomboid fossa) and tent-like roof, orienting you to the median sulcus, sulcus limitans, and the paired facial colliculi on the pontine portion of the floor. As the sequence progresses, the ventricular roof is defined by the superior and inferior medullary vela and tela choroidea, with the choroid plexus projecting into the ventricular lumen. Lateral recesses extend inferolaterally toward the cerebellopontine angle, ending at the foramina of Luschka, while the midline foramen of Magendie opens inferiorly toward the cisterna magna. These relationships matter when you are localizing brainstem signs. Lesions that compress the dorsal pons or medulla from within the ventricle, such as ependymoma, medulloblastoma with fourth-ventricular extension, or obstructive clot, can distort the rhomboid fossa and impede CSF outflow, producing noncommunicating hydrocephalus.
