Rotatores Lumborum

Description

Running obliquely between adjacent lumbar vertebrae, the rotatores lumborum sit deepest in the posterior back, spanning from the transverse process of a lower vertebra to the lamina and base of the spinous process above. A posterior view places them medial to the long columns of erector spinae and immediately deep to multifidus, with their fibers crossing the intertransverse space close to the zygapophysial (facet) joints. Inferiorly, the muscular field blends toward the sacrum and posterior iliac crest, where the thoracolumbar fascia forms a dense aponeurotic roof over the paraspinal compartment; laterally, latissimus dorsi and the abdominal obliques frame the lumbar triangle, and the upper gluteal mass anchors the pelvis. The rotatores are often taught as part of the transversospinalis group because their short lever arm favors segmental control and proprioception rather than torque production, which is why this deep-layer dissection is useful when you need to explain lumbar stability beyond the superficial erector spinae. Their proximity to the facet capsules and medial branches of the dorsal rami makes them relevant in facet-mediated low back pain, medial branch blocks, and posterior approaches for decompression or fusion where splitting multifidus can denervate and atrophy the deep paraspinals.
