Description
Centered on the external nose in an anterior head and neck view, the anterior dilator naris appears as small subcutaneous muscle slips along the anterolateral rim of the nostril, just superficial to the lateral crus of the alar cartilage and lateral to the nasal septum. Fibers blend medially with the alar part of the nasalis and laterally with adjacent perinasal soft tissue over the maxilla, placing the muscle inferior to the nasal bones and superior to the upper lip. Nearby, the levator labii superioris alaeque nasi runs vertically along the side of the nose, while the orbicularis oris forms a muscular ring inferiorly around the oral aperture. Surface anatomy matters here. Functionally, the anterior dilator naris contributes to nostril flare and fine control of the nasal valve region, a narrow segment where small changes in soft tissue tension can alter airflow resistance during exertion. Surgeons reference these fiber bundles when planning rhinoplasty or alar base reduction because overresection, scarring, or disruption of the perinasal muscular insertions can worsen external nasal valve collapse and contribute to postoperative obstruction. Denervation from facial nerve (temporal and zygomatic-buccal branches to perinasal musculature) can also blunt symmetric alar dilation, an exam finding that helps localize facial palsy severity beyond the smile. Ideal for facial anatomy teaching in head and neck dissection labs, ENT and plastic surgery atlases, and patient-facing explanations of functional rhinoplasty concepts, this anterior perspective clarifies how a small intrinsic nasal muscle sits relative to cartilage, septum, and the upper lip elevators.
