Anterior Perspective of the Larynx

Description

Prominent in anterior view, the paired laminae of the thyroid cartilage meet at the midline to form the laryngeal prominence (Adam’s apple), with the superior thyroid notch lying just inferior to the body of the hyoid bone. Superiorly, the thyrohyoid membrane spans between hyoid and thyroid cartilage, while inferiorly the cricothyroid ligament bridges the gap to the arch of the cricoid cartilage. The cricothyroid muscle occupies the anterolateral surface, running from the anterolateral cricoid to the inferior border and horn of the thyroid cartilage, and the first tracheal rings continue inferiorly in the midline as the airway transitions into the cervical trachea. Small infrahyoid strap muscles are suggested superficially, with their fibers oriented vertically along the anterior neck. For teaching laryngeal mechanics, this perspective clarifies the only external intrinsic laryngeal muscle, the cricothyroid, and its action of tilting the thyroid cartilage anteriorly relative to the cricoid to lengthen and tense the vocal folds. That relationship underpins clinical maneuvers such as cricothyrotomy, where the cricothyroid membrane is accessed in the midline between thyroid and cricoid cartilages, and also explains why thyroid cartilage angle and prominence are typically more acute and visible in adult males.
