Description
Anterior to the knee joint, the patella sits within the quadriceps tendon and continues inferiorly as the patellar ligament to its insertion on the tibial tuberosity. Medial and lateral femoral condyles frame the trochlear groove superior to the patella, while the proximal tibial plateau lies posterior to the ligament’s broad, flattened band. Collateral ligament contours can be inferred along the medial and lateral margins of the joint line, and the tibial crest runs distal to the tuberosity. Straightforward anatomy. Clinically, this anterior perspective is the one you want when teaching the knee extensor mechanism and the biomechanics of patellar tracking, because the patellar ligament defines the line of pull from quadriceps to tibia and sets up the patellar tendon reflex arc at the level of L2 to L4. The tibial tuberosity is a palpable landmark and a frequent pain generator in Osgood-Schlatter disease, while degenerative patellar tendinopathy and partial tears cluster near the inferior pole of the patella in jumping athletes. Surgical approaches that use a midline anterior incision, including patellar tendon autograft harvest for ACL reconstruction, also hinge on understanding the ligament’s width, edges, and relationship to the prepatellar and infrapatellar soft tissues.
