Description
Anteriorly sectioned uterus and adnexa are presented with the endometrial cavity and myometrium opened to reveal the relationship of the uterine corpus to the bilateral adnexal structures. Lateral to the uterine fundus, the animation tracks along the fallopian tube toward the ovary, where a well-circumscribed ovarian endometrioma (chocolate cyst) expands within the ovarian cortex and displaces normal stromal tissue peripherally. Subtle sequential motion clarifies depth in the cut face, keeping the ovary in continuity with the tubo-ovarian region and broad ligament attachments. Clinically, an ovarian endometrioma is a common manifestation of endometriosis and a frequent source of chronic pelvic pain, dysmenorrhea, dyspareunia, and infertility. The stepwise reveal helps you appreciate how the cyst can distort tubo-ovarian anatomy and contribute to adhesions around the fimbriae, a key mechanism for reduced oocyte pickup and altered tubal patency. It also sets up surgical thinking: cystectomy planes at the interface of the cyst wall and ovarian cortex, and the trade-off between complete excision and preservation of ovarian reserve.
