Description
Anteriorly, the fibrous articular capsule of the knee forms a sleeve around the distal femur and proximal tibia, attaching proximally near the femoral condyles and distally along the margins of the tibial plateau. The patella sits anterior to the femoral trochlea within the quadriceps tendon, and the capsule blends with the patellar retinacula as it approaches the medial and lateral aspects of the joint. Deep to the fibrous layer, the synovial membrane reflects to line the nonarticular surfaces, leaving the articular cartilage of the femoral condyles, tibial plateau, and patellar facets uncovered. Patellar cartilage is highlighted. This anterior capsule-focused view matters because the knee’s anterior soft tissue envelope is where exam findings, injections, and surgical portals converge. Standard arthroscopy portals (anterolateral and anteromedial, adjacent to the patellar tendon) traverse capsule and synovium, so understanding the patellar tendon, retinacular expansions, and capsular attachment lines helps explain why malposition can irritate the infrapatellar (Hoffa) fat pad or miss the suprapatellar recess.
