Description

Arising deep to the inguinal ligament, the external iliac vein continues proximally from the femoral vein and ascends along the pelvic brim toward the common iliac vein. From an anterior perspective in the male pelvis, the vessel lies medial to the external iliac artery and courses on the medial margin of the psoas major, with the iliacus muscle forming a lateral floor. Inferiorly, the femoral vein enters the field beneath the inguinal ligament, while the superior pubic ramus and adjacent pelvic bones give fixed landmarks for the vessel’s position. Clinical relevance centers on the external iliac vein as the principal outflow tract from the lower limb, a frequent site of iatrogenic injury during anterior approaches to the acetabulum and pelvic brim fixation, and a key structure when mobilizing the iliac vessels in pelvic and transplant surgery. Catheter-based procedures also pass through this corridor, and the close apposition of vein and artery helps explain why pelvic hematoma, lymphocele, or postoperative scarring can produce venous compression with unilateral leg swelling.
