Description
Centered in the anterior skull base, the ethmoid bone is presented from the front with the perpendicular plate descending in the midline toward the nasal cavity. Superior to it, the cribriform plate spans horizontally, while the ethmoidal labyrinths expand laterally as paired masses that contribute to the medial orbital walls and the upper lateral nasal wall. Along the anterior surface, the bony contours that frame the ethmoidal air cells sit medial to the orbits and superior to the nasal cavity. Midline anatomy reads clearly. Clinical attention often fixes on the cribriform plate and adjacent labyrinth because this is the corridor where sinonasal disease meets the anterior cranial fossa. A fracture through the cribriform plate can shear olfactory nerve filaments and open a route for cerebrospinal fluid rhinorrhea, while ethmoid sinusitis can erode thin bony partitions and spread to the orbit through the lamina papyracea, producing orbital cellulitis or a subperiosteal abscess. Endoscopic sinus surgery also uses the perpendicular plate and superior septal region as midline landmarks when working near the skull base. Use this anterior ethmoid view when teaching nasal septum formation (perpendicular plate contribution) in gross anatomy, head and neck, or neuroanatomy blocks, and when illustrating the relationship of ethmoidal air cells to the orbit for ENT texts and surgical atlases.
