Description

Viewed from anterior, the female coxal region centers on the acetabulum of the os coxae receiving the femoral head, with the femoral neck running inferolaterally toward the greater trochanter. Superiorly, the anterior superior iliac spine and anterior inferior iliac spine bracket the anterolateral pelvis, while the pubic body and superior pubic ramus sit more medial and inferior. The greater trochanter forms the lateral contour of the proximal femur, and the lesser trochanter lies posteromedial to it, only partly appreciable from the front. Iliopsoas tracks anterior to the hip capsule toward the lesser trochanter, while rectus femoris originates near the AIIS and crosses superficially. Orientation is clear. Clinically, an anterior understanding of the hip joint helps when teaching why femoroacetabular impingement often presents with groin pain and a positive FADIR test, since the conflict occurs at the anterosuperior femoral head neck junction and acetabular rim. This is also the perspective surgeons use to plan the direct anterior approach for total hip arthroplasty, working between tensor fasciae latae and sartorius/rectus femoris while staying mindful of the lateral femoral cutaneous nerve near the ASIS and the femoral neurovascular bundle medial to the iliopsoas. Landmarks around the greater trochanter matter when differentiating trochanteric pain from true intra-articular pathology.
