Palmar skin creases and pads dominate this anterior view of a female palm, with the thenar eminence lying radial and proximal at the base of the thumb and the hypothenar eminence forming the ulnar and proximal border near the little finger. Distal to these muscular masses, the central palm overlies the metacarpus, with the metacarpal heads and their associated palmar fascia aligning proximal to the digits. The longitudinal axis of the hand runs from the carpal region proximally toward the digital rays distally, and the palmar surface presents thick glabrous skin anchored to the underlying palmar aponeurosis. Orientation is unmistakably anterior (palmar). Clinically, the contour of the thenar region is a fast screen for median nerve function, since denervation from carpal tunnel syndrome or recurrent motor branch injury produces thenar flattening and loss of thumb opposition. Ulnar-sided hypothenar bulk and the adjacent superficial palmar structures matter when teaching ulnar neuropathy and when planning incisions that avoid the superficial palmar arch and common digital nerves during palmar laceration repair.
