Centered on the anterior orbit, the palpebral fissure forms the elliptical interpalpebral aperture between the superior and inferior eyelids, bordered by the lid margins and eyelashes. Medially, the fissure tapers toward the medial canthus where the lacrimal caruncle and plica semilunaris typically sit just lateral to the lacrimal lake, while laterally it ends at the lateral canthus with a sharper angle. The upper lid normally overlaps the superior limbus in primary gaze, and the lower lid rests near the inferior limbus, so subtle differences in palpebral fissure height can be appreciated in a straight-on view. Small landmarks matter. Anterior assessment of the palpebral fissure is a workhorse for documenting eyelid position and symmetry, because margin-reflex distance (MRD1 and MRD2), canthal tilt, and scleral show can all be judged relative to the corneal light reflex and limbus. Ptosis from levator aponeurosis dehiscence, Horner syndrome, or third nerve palsy alters the superior lid contour, while facial nerve palsy leads to lagophthalmos and exposure keratopathy with widening of the interpalpebral space. Thyroid eye disease often increases palpebral fissure height through upper lid retraction, and cicatricial or involutional ectropion/entropion changes the lid margin apposition along the inferior aspect, shifting the tear meniscus and driving epiphora or ocular surface irritation. Ophthalmology and head and neck anatomy courses use this view to teach eyelid surface anatomy, canthal relationships, and basic clinical measurements before students move on to slit-lamp biomicroscopy and oculoplastics.
