Description
Arising as multiple hypoglossal rootlets from the preolivary sulcus of the medulla oblongata, the twelfth cranial nerve is shown converging into a single trunk that courses anterolaterally toward the upper neck. Distally, the nerve approaches the tongue from an inferior and lateral position, distributing motor branches across the intrinsic tongue musculature and into the extrinsic muscles, with genioglossus medial, hyoglossus more lateral and inferior, and styloglossus posterolateral in expected relationships. Midline symmetry of the tongue emphasizes paired innervation patterns and the compartmentalization of muscle fiber directions across the median septum. Adjacent neurovascular structures are suggested in context, with lingual vessels running in the floor of mouth near the hyoglossus plane where the hypoglossal nerve typically travels. An anterior perspective is practical when teaching how a brainstem lesion differs from a peripheral hypoglossal injury, because it anchors the nerve to its medullary origin before following it to the tongue. Unilateral hypoglossal palsy produces ipsilateral tongue weakness and deviation toward the affected side on protrusion, a direct consequence of genioglossus imbalance.
