Description
Along the ulnar (medial) border of the female palm, the hypothenar eminence forms a muscular pad superficial to the fifth metacarpal and proximal phalanx of the little finger. An anterior view brings forward the hypothenar muscle group, abductor digiti minimi lying most ulnarly, flexor digiti minimi brevis positioned more radial and anterior to the fifth metacarpal, and opponens digiti minimi coursing deep to them along the shaft to rotate the metacarpal at the carpometacarpal joint. Distally, these fibers converge toward the ulnar side of the base of the proximal phalanx and the extensor expansion, while proximally they relate to the palmar aponeurosis and the pisiform and hook of hamate region where their tendinous origins concentrate. The surface contour also implies the course of the ulnar neurovascular bundle entering the hand in Guyon’s canal just radial to the pisiform before dividing into superficial and deep branches that supply this compartment. Clinically, the hypothenar eminence is where ulnar neuropathy and ulnar artery injury declare themselves in the palm. Think handlebar palsy. Compression at Guyon’s canal can selectively weaken abduction and opposition of the little finger by denervating abductor digiti minimi and opponens digiti minimi, while sparing dorsal ulnar cutaneous sensation because that branch leaves the ulnar nerve proximal to the wrist. The same topography guides incision planning for hypothenar fat pad flaps and helps distinguish local muscle atrophy from generalized intrinsic wasting seen in advanced ulnar nerve lesions at the elbow.
