Beneath a semi-transparent cutaneous layer, the anterior face is rendered to emphasize the depressor labii inferioris (also referred to in older texts as quadratus labii inferioris) in the lower perioral region of an adult male. The muscle originates from the oblique line of the mandible and ascends superiorly and medially to blend with the orbicularis oris and the lower lip, lying lateral to the mentalis at the midline chin. Surrounding expression muscles are also visible in anatomical position, including orbicularis oculi circumferential to the palpebral fissures, zygomaticus major and minor running inferomedially from the zygomatic bone toward the modiolus, buccinator deep to the cheek, and corrugator supercilii at the glabella. Depression and eversion of the lower lip depend on the balance between depressor labii inferioris, mentalis, and orbicularis oris, so this layered view helps clarify why perioral asymmetry can follow mandibular or lower lip surgery. Injury to the marginal mandibular branch of the facial nerve (CN VII), often at risk during submandibular approaches or facelift dissection near the inferior border of the mandible, can weaken depressor labii inferioris and produce an uneven smile with lower lip incompetence.
