Description
Across the anterior knee, the patella sits within the extensor mechanism, aligned superiorly with the distal femur and inferiorly with the proximal tibia. Lateral to the patellar margin, the lateral patellofemoral ligament spans from the lateral femoral epicondylar region toward the superolateral patella, forming a broad, obliquely oriented restraint across the patellofemoral joint line. Adjacent structures expected in this projection include the lateral femoral condyle deep to the ligament, the tibial plateau inferiorly, and the lateral collateral ligament more laterally, coursing toward the fibular head. Lateral soft tissue restraint matters when you are explaining patellar tracking and instability. The lateral patellofemoral ligament blends with the lateral retinaculum and influences lateral patellar tilt, so it becomes clinically relevant in lateral retinacular tightness, post-traumatic maltracking, and in the planning or critique of a lateral retinacular release, which can destabilize the patella if performed indiscriminately. Small structure, big consequences. For teaching, this anterior view works well in knee anatomy and sports medicine modules where learners must distinguish patellofemoral stabilizers from the intra-articular cruciate ligaments and the tibiofemoral collateral ligaments. It also suits operative planning discussions in orthopaedics and rehabilitation resources that address recurrent patellar dislocation, anterior knee pain, and patellofemoral cartilage overload patterns.
