Description
Across the distal anterior forearm, the radius lies lateral and the ulna medial, tapering into the carpus where the palpable pisiform and hook of hamate sit on the ulnar side and the scaphoid and trapezium occupy the radial column. Superficial flexor tendons course distally toward the palm, with the palmaris longus tendon, when present, tracking centrally toward the palmar aponeurosis and the flexor carpi radialis tendon running just lateral to it. The volar wrist crease overlies the radiocarpal region, while the thenar eminence rises proximally on the radial side and the hypothenar eminence defines the ulnar border of the proximal palm. Skin landmarks matter. An anterior wrist view is the working surface for vascular access, tendon assessment, and many hand surgery approaches, so the relationship between surface creases and deeper structures has practical consequences. The median nerve and flexor tendons pass deep to the flexor retinaculum within the carpal tunnel, while the ulnar nerve and artery traverse Guyon canal superficial and ulnar to the retinaculum near the pisiform, a distinction that guides examination of carpal tunnel syndrome versus ulnar tunnel neuropathy. Palpation of the flexor carpi radialis tendon also helps localize the radial artery just lateral to it during arterial line placement and Allen test teaching.
