Description

Centered in the anterior abdominal wall, the rectus abdominis borders the midline linea alba, while the external oblique forms the lateral contour of the trunk. Deep to these muscles, the peritoneum appears as a continuous serosa lined by mesothelium, reflecting from the parietal peritoneum on the inner abdominal wall to the visceral peritoneum investing the stomach and the convoluted small intestine. Superiorly the peritoneal reflections relate to the inferior surface of the diaphragm, and inferiorly they extend into the pelvic cavity in the male, where the peritoneum forms the rectovesical pouch between the urinary bladder and rectum. The colon frames the central small-bowel loops, with peritoneal folds positioning the large intestine relative to the posterior abdominal wall. Anterior peritoneal anatomy matters when you are teaching why certain organs are intraperitoneal (mobile, invested by visceral peritoneum) while others sit secondarily retroperitoneal, a distinction that predicts the spread of infection and the pattern of referred pain. Free intraperitoneal air from a perforated peptic ulcer rises to the subdiaphragmatic recesses, while ascites tracks along the paracolic gutters, and both phenomena become easier to conceptualize when the serosal lining and its reflections are shown in a body-positioned, front-facing layout. A clean peritoneal map also supports operative orientation for laparoscopic access through the anterior abdominal wall, where trocars traverse skin, fascia, and parietal peritoneum before entering the peritoneal cavity.
