Sweeping from the superior cerebellar hemispheres toward the dorsal midbrain, the superior cerebellar peduncle (brachium conjunctivum) appears as a paired, compact white matter stalk flanking the upper fourth ventricle and roof of the rhomboid fossa. An anterior perspective places the peduncles medial to the cerebellar hemispheres and inferior to the tectal region, converging as they ascend to the level of the inferior colliculi. The sequence tracks their course rostrally into the mesencephalon, clarifying where the fibers decussate within the caudal midbrain before continuing toward the red nucleus and thalamic targets. Most cerebellar efferents exit through the superior cerebellar peduncle, so its topography matters when localizing ataxia, dysmetria, and intention tremor from brainstem stroke, demyelination, or tumor compression. Lesions at or below the decussation tend to produce ipsilateral cerebellar signs, while involvement above the decussation shifts deficits contralaterally, a point that often confuses trainees. Seeing the peduncle move in relation to the fourth ventricle and midbrain landmarks helps explain why dorsal midbrain and superior pontine lesions can mimic primary cerebellar disease.
