Description
Viewed from inferiorly just deep to the skin, the paired digastric muscles frame the submental region beneath the mandible, with the anterior bellies running posteroinferiorly from the digastric fossae toward the midline and the posterior bellies coursing anteroinferiorly from the mastoid notch toward the hyoid. Each side converges on an intermediate tendon that is tethered to the body and greater horn of the hyoid by a fibrous sling, placing the tendon inferior to the mandibular arch and superior to the laryngeal inlet. Medial to the anterior bellies you would expect the mylohyoid forming the muscular floor of mouth, while deeper midline fibers align with geniohyoid; posteriorly, the epiglottis and thyroid cartilage sit superior to the first tracheal rings. Symmetry matters here. Surgeons use the digastric as a landmark in the suprahyoid neck because its borders define the submental and submandibular triangles, guiding exposure of level IA and IB lymph nodes during selective neck dissection. The posterior belly also helps orient you to the carotid sheath region and nearby cranial nerves, and the intermediate tendon at the hyoid is a reliable reference when assessing suprahyoid dysfunction in dysphagia and after mandibular or hyoid trauma. In submandibular gland excision, identifying the anterior belly and mylohyoid junction reduces the chance of drifting toward the hypoglossal nerve and lingual nerve in the floor of mouth.
