Description
Oriented from an inferior perspective, the terminal inferior mesenteric artery descends in the left posterior abdomen and continues into the pelvis as the superior rectal artery, running along the posterior aspect of the sigmoid colon toward the upper rectum. Proximal branches consistent with the left colic artery and multiple sigmoid arteries fan laterally and superiorly within the sigmoid mesocolon to reach the descending and sigmoid colon. Haustra and the longitudinal teniae coli mark the colonic wall, while accompanying blue venous channels track parallel to the arterial arcade in the mesentery. Small pericolic and intermediate lymph nodes appear as yellow nodules clustered along the vascular pedicle and adjacent bowel. An inferior view clarifies where the inferior mesenteric artery transitions into its final rectal supply, a relationship that matters during low anterior resection and total mesorectal excision when you are balancing oncologic clearance against preservation of perfusion to the remaining colon and rectal stump. This is the territory where high versus low ligation of the IMA influences collateral flow through the marginal artery of Drummond and can contribute to anastomotic ischemia if the sigmoid and superior rectal branches are sacrificed without adequate collateralization.
