Centered in the suprahyoid neck, the submental triangle sits in the midline just superior to the body of the hyoid bone, bordered laterally by the right and left anterior bellies of the digastric muscles and inferiorly by the hyoid. The mylohyoid forms the muscular floor of this space, while the platysma and superficial cervical fascia lie more superficial and the geniohyoid and genioglossus are positioned deep and superior toward the mandibular symphysis. Submental lymph nodes typically occupy the subcutaneous tissue overlying the mylohyoid, with the paired anterior jugular veins running inferiorly near the midline. Small but busy territory. Clear orientation of the submental triangle matters when evaluating anterior neck swelling and when planning midline approaches to the floor of mouth, because the differential often turns on what lies above versus below the mylohyoid. Infection from the mandibular incisors, lower lip, or anterior floor of mouth can drain to the submental nodes and then track across fascial planes, a pathway that underpins the early spread patterns seen in odontogenic infection and Ludwig angina. This region also frames the submental artery perforator plane used in submental flap reconstruction and helps explain why the anterior jugular veins are at risk during superficial dissection.
