Description

Inferior to the hyoid bone in the anterior cervical region, the infrahyoid (strap) muscles are arranged as paired vertical bands flanking the midline. Sternohyoid lies most superficial and medial, while omohyoid crosses obliquely with its superior belly approaching the hyoid and its inferior attachment coursing toward the scapular region, separated by an intermediate tendon. Deeper, sternothyroid ascends from the posterior manubrium to the thyroid cartilage, and thyrohyoid continues superiorly from the thyroid cartilage to the hyoid, sitting immediately anterior to the laryngeal framework. Midline relationships are anchored by the laryngeal prominence and the adjacent thyroid cartilage. For teaching surface and surgical anatomy, this region matters because the strap muscles form the first muscular layer encountered when approaching the larynx and thyroid gland from the front of the neck. Their separation along the linea alba cervicalis and retraction laterally is a standard step in thyroidectomy and tracheostomy, and their bulk influences exposure of the pretracheal fascia, tracheal rings, and the thyroid isthmus. Their innervation through the ansa cervicalis (C1 to C3), with C1 fibers traveling briefly with the hypoglossal nerve, also makes this a useful map for understanding postoperative dysphonia patterns when retraction or scarring affects adjacent laryngeal mechanics.
