Obliquely from the lateral aspect, the adult male liver is oriented to emphasize the anterior medial segment (Couinaud segment V) on the anteroinferior surface of the right hepatic lobe, bounded medially by the interlobar plane toward the gallbladder fossa and laterally by the more peripheral right anterior sector. Superiorly, segment V continues toward segment VIII, while inferiorly it approaches the inferior border of the liver near the costal margin. The relationship to the middle hepatic vein plane is implied by the segmental designation, even when the venous trunks themselves are not foregrounded. Segmental anatomy matters. Couinaud segment V is a workhorse landmark for hepatobiliary surgery because it sits adjacent to the gallbladder bed and the right portal pedicle distribution, making it a frequent target in segmentectomy, nonanatomic wedge resection, and operative planning for hepatocellular carcinoma or colorectal liver metastases. A lateral view helps orient the surgeon and trainee to what lies on the surface versus what must be inferred from portal and hepatic venous territories, a common source of wrong-segment localization when working from axial imaging alone.
