Description
Posterior thoracic skeleton is presented with the ribs emphasized against the thoracic vertebrae, so the costovertebral and costotransverse joints read clearly at each rib head and tubercle. Rib shafts sweep laterally and anteriorly from the vertebral column, forming the costal curvature and defining the posterior rib angles just lateral to the transverse processes. Superiorly, the occipital squama and upper cervical vertebrae frame the top of the field, while the scapulae lie posterolateral to the rib cage and partially overlap the upper ribs; the proximal humeri sit lateral to the scapular glenoid regions. Landmarks are easy to orient. Posterior rib anatomy matters because many clinically relevant injuries and procedures reference rib angles, vertebral levels, and the intercostal spaces in this region. Blunt trauma often produces fractures near the rib angle, and posterior displacement can threaten the pleura or create pain patterns that track along the intercostal neurovascular bundle (VAN) in the costal groove on the inferior border of each rib. For thoracic surgery and regional anesthesia, accurate rib numbering from the spine is the starting point for planning posterolateral thoracotomy levels, chest tube placement relative to the scapula, and paravertebral or erector spinae plane blocks that aim near the transverse processes and costotransverse ligaments. Small differences in thoracic kyphosis and scapular position can also change surface relationships. Use this posterior ribcage view in gross anatomy and osteology teaching to reinforce rib morphology, rib counting, and joint relationships, or in clinical skills materials that explain posterior thoracic landmarks for trauma evaluation and procedural approaches.
