Description
Anterior surface anatomy centers on the interval between the lateral border of pectoralis major and the anterior margin of deltoid, forming the deltopectoral groove (Mohrenheim’s fossa) just inferior to the clavicle. Superiorly the groove opens into the deltopectoral triangle, bounded by the clavicle, deltoid, and pectoralis major, with the coracoid process lying deep and slightly medial. Within this sulcus the cephalic vein typically ascends proximally toward the infraclavicular region, piercing the clavipectoral fascia to join the axillary vein. Cutaneous contours reflect the female anterior chest wall, including the superficial curve overlying pectoralis major toward the breast. Clinically, this landmark matters because the deltopectoral interval is a standard anterior approach to the shoulder, used for open reduction and internal fixation of proximal humerus fractures and for shoulder arthroplasty, where the surgeon works between deltoid laterally and pectoralis major medially to protect the axillary nerve and preserve deltoid function. The cephalic vein in the groove supports venous cutdown and is commonly used for lead insertion during pacemaker or ICD implantation through a deltopectoral incision. Needle and scalpel placement in this region also demands respect for the deeper thoracoacromial vessels beneath the clavipectoral fascia and the proximity of the axillary artery and brachial plexus as you move laterally and inferiorly.
