Centered above the clavicle, the lesser supraclavicular fossa appears as a small triangular depression between the sternal and clavicular heads of the sternocleidomastoid muscle, immediately superior to the medial third of the clavicle and lateral to the jugular (suprasternal) notch. Superficial layers would include skin and platysma, with the investing layer of deep cervical fascia outlining the contour as it spans from the manubrium and clavicle toward the mastoid process. Deep to this surface landmark lie the scalene muscle compartment and the cervicothoracic junction, where the subclavian vein courses anterior to the anterior scalene and the subclavian artery and brachial plexus pass posterior to it. Palpation and surface marking in this region matter because small shifts in probe or needle angle can change what you are approaching from venous structures to pleura. The medial supraclavicular area sits near the dome of the pleura and cupola, a relationship that explains pneumothorax risk during low neck and infraclavicular procedures and after clavicular trauma. Supraclavicular lymph nodes also concentrate along the inferior deep cervical chain near the clavicle, so focal fullness in this fossa can support clinical suspicion for metastatic spread, including left-sided Virchow node enlargement. Teach gross anatomy students to distinguish the lesser from the greater supraclavicular fossa and to identify the two heads of sternocleidomastoid as a landmark for central venous access planning, cervical fascial planes, and neck examination technique. It also fits well in interventional radiology and anesthesia teaching files when discussing catheter trajectories, pleural avoidance, and supraclavicular approaches to the brachial plexus.
