Description
Anterior crural anatomy is presented from the ventral aspect of an adult male lower leg, centered on the anterior compartment between the tibial crest medially and the fibula laterally. Tibialis anterior lies closest to the tibia and runs distally toward the medial cuneiform and base of the first metatarsal, while extensor hallucis longus occupies a deeper, more central plane as it tracks toward the hallux. Extensor digitorum longus sits more laterally, its muscle belly giving way to tendons that pass anterior to the ankle toward digits two through five, all invested by the deep fascia (fascia cruris) and organized under the extensor retinacula at the distal leg. Clean lines. Clear separations. This anterior presentation matters because the compartment’s tight fascial boundaries make it the classic site for acute anterior compartment syndrome after tibial shaft fracture or reperfusion, with early pain on passive plantarflexion and progressive weakness of dorsiflexion. The course of the deep fibular (peroneal) nerve and anterior tibial artery through this compartment also frames common iatrogenic and entrapment concerns, including injury during anterior approaches to the ankle and sensory loss in the first dorsal web space. For teaching tendon mechanics, this view offers an efficient map of how dorsiflexors transition to long tendons and why anterior ankle swelling can quickly become functionally limiting.
