Description

Oriented to the anterior neck of an adult woman, the carotid triangle is bounded inferiorly by the superior belly of the omohyoid, posterolaterally by the anterior border of the sternocleidomastoid, and superiorly by the posterior belly of the digastric (often with the stylohyoid running parallel). Within those surface limits, the carotid sheath region lies deep to platysma and the investing layer of deep cervical fascia, with the hyoid bone and thyroid cartilage providing midline reference. The triangle’s floor is formed by the thyrohyoid, hyoglossus, and middle and inferior pharyngeal constrictors, sloping posteromedially toward the pharynx. Palpation and surgery both depend on these boundaries because they guide a safe path to the common carotid artery and its bifurcation into internal and external carotid arteries, where the carotid sinus and carotid body sit at the posteromedial aspect of the split. This is the operative corridor for carotid endarterectomy and for exposure of the external carotid artery and its branches (commonly the superior thyroid artery), and it is also where an overly medial dissection risks the hypoglossal nerve as it crosses the carotids near the digastric tendon, while inferior traction can endanger the ansa cervicalis on the anterior wall of the sheath. Know this triangle. It also frames bedside maneuvers such as locating the carotid pulse and performing carotid sinus massage, and it explains why high internal jugular venous cannulation can lead to inadvertent arterial puncture if landmarks are misread.
