Description

Across the anterior male thorax, the precordium (regio praecordialis) is defined as the area of the chest wall immediately superficial to the heart and great vessels. The region lies left of the median plane, spanning the parasternal area and adjacent left anterior chest over the body of the sternum and the costal cartilages of ribs 2 through 6, with its inferior limit approaching the infrasternal angle and superiorly nearing the sternal angle. Medially it abuts the sternum, laterally it blends into the left mammary region, and deep to it sit the pericardium, right ventricle (most anterior), left ventricle, and the proximal ascending aorta and pulmonary trunk. Precordial mapping matters because it ties surface anatomy to bedside examination and procedural safety. The standard auscultation points are distributed around this zone: the aortic area at the right second intercostal space, the pulmonic area at the left second intercostal space, Erb point at the left third intercostal space, the tricuspid area along the lower left sternal border, and the mitral area near the left fifth intercostal space at the midclavicular line. These landmarks guide evaluation of murmurs, pericarditis (often pleuritic pain with a friction rub best heard at the left sternal border), and urgent interventions such as pericardiocentesis, where parasternal and subxiphoid approaches are planned to avoid internal thoracic vessels and pleura.
