Description
Arising lateral to the ala of the nose, the nasolabial sulcus (melolabial fold) descends obliquely toward the oral commissure, forming the visible boundary between the cheek and the upper lip in a male adult profile. Superiorly it relates to the alar base and the nasolabial portion of the levator labii superioris alaeque nasi, while inferiorly it approaches the modiolus where fibers of orbicularis oris, zygomaticus major, and depressor anguli oris interlace. The groove lies medial to the malar fat pad and lateral to the philtrum, deepening where mimetic musculature tethers skin to deeper fascia. A familiar facial landmark. Surface conformation of the nasolabial fold matters because it is where facial expression, age-related volume loss, and skin fixation become clinically legible. With aging, descent of the malar fat pad and attenuation of subcutaneous support exaggerate the lip-cheek groove, a common target in facial aesthetic consultation for hyaluronic acid filler placement, autologous fat transfer, and midface lift planning. It also functions as a practical reference line when discussing facial nerve branches and the perioral danger zone, where over-aggressive dissection or injection can injure the facial artery as it courses toward the angular artery near the alar region.
