Description
Anterior deep exposure into the right ventricle brings the trabeculated cavity into view, with the musculi papillares dextri projecting as conical columns from the ventricular wall. Chordae tendineae extend from the papillary tips to the anterior, posterior, and septal leaflets of the tricuspid valve, spanning the chamber toward the atrioventricular orifice. Superiorly, the trabecula septomarginalis (moderator band) often crosses from the interventricular septum to the anterior papillary muscle, while the infundibulum leads cranially toward the pulmonary valve. Functionally, this arrangement prevents tricuspid leaflet prolapse during systole by tensioning the chordae rather than actively opening the valve. Papillary muscle rupture or elongation after right ventricular infarction, blunt chest trauma, or infective endocarditis can produce acute severe tricuspid regurgitation, and this cutaway view makes the leaflet chordal attachments and their vectors of pull easy to teach. Surgeons also care about these relationships when placing or revising tricuspid annuloplasty rings and when repairing chordae, because sutures placed too deep along the septal side risk proximity to the membranous septum and the atrioventricular conduction bundle.
