Centered over the cranial vault, the epicranium is rendered as the scalp’s musculoaponeurotic layer, with the galea aponeurotica (epicranial aponeurosis) spanning between the frontal belly and occipital belly of the occipitofrontalis (epicranius) muscle. Anteriorly, the frontalis fibers blend inferiorly toward the superciliary region, while posteriorly the occipitalis anchors to the superior nuchal line and mastoid region. Laterally, the aponeurosis thins toward the temporal lines where it relates to the superior auricular region and transitions toward temporoparietal fascia superficial to the temporalis fascia. The scalp layers deep to the epicranium, loose areolar tissue and pericranium, sit in series over the calvaria. Clean anatomy. Understanding the epicranial aponeurosis matters any time you elevate a scalp flap or plan incisions across the vertex, because the galea and its attachments determine flap mobility and the direction tension will transmit to the forehead and occiput. Scalp lacerations that traverse the galea tend to gape and bleed briskly due to the tethering of the dense connective tissue layer and the rich subcutaneous vascular plexus supplied by branches of the superficial temporal and occipital arteries. Surgeons also track this plane when raising coronal approaches for craniofacial exposure and when managing subgaleal hematoma, which can spread widely in the loose areolar layer deep to the aponeurosis.
