Description
Anterior cervical anatomy is centered on the muscular triangle (Trigonum musculare, omotracheal triangle), bounded laterally by the anterior border of the sternocleidomastoid, superolaterally by the superior belly of the omohyoid, and superiorly by the anterior belly of the digastric and its intermediate tendon via the hyoid level. Within this inferior, medial subdivision of the anterior triangle, the infrahyoid strap muscles dominate the field: sternohyoid and sternothyroid run vertically toward the hyoid and thyroid cartilage, while the thyrohyoid continues superiorly from the thyroid cartilage to the hyoid. Deep to the strap layer, the laryngeal framework and upper trachea sit in the midline, with the thyroid gland occupying anterolateral positions around the proximal trachea and below the thyroid cartilage. Surgical anatomy lives here. A low anterior approach to the airway, including tracheostomy and cricothyrotomy planning, depends on recognizing how the sternohyoid and sternothyroid are separated in the median raphe, retracted laterally, and how the thyroid isthmus may overlie the second to fourth tracheal rings and require division or mobilization. Relationships to the carotid sheath are implied at the muscular triangle’s lateral boundary, a reminder that staying medial protects the common carotid artery, internal jugular vein, and vagus nerve. Short and consequential.
