Description
Seen in lateral profile, the superior palpebra (upper eyelid) forms a thin, layered curtain from the supraorbital skin crease down to the free lid margin as it apposes the anterior surface of the globe. Superficial skin and subcutaneous tissue overlie the orbicularis oculi, which sits anterior to the dense tarsal plate and the palpebral conjunctiva lining the posterior surface. Superiorly and posteriorly, the levator palpebrae superioris aponeurosis fans forward to insert into the anterior tarsus and skin, while the superior tarsal muscle (Müller muscle) bridges from the levator complex to the superior border of the tarsus. Laterally, the lid margin sweeps toward the lateral canthus, where the lateral palpebral raphe and canthal tendon anchor the eyelid to the lateral orbital rim. Layer recognition matters in eyelid surgery because millimeters decide function. A lateral view clarifies how levator dehiscence or attenuation produces aponeurotic ptosis, and why levator advancement targets the aponeurosis rather than the tarsus itself. It also helps explain common postoperative problems after upper blepharoplasty, including lagophthalmos from excessive skin or orbicularis resection and dry eye exacerbation when lid-globe apposition is altered. Small anatomy, big consequences.
