Description
Posteriorly, the male posterior axillary fold is defined by the inferolateral border of the latissimus dorsi as it sweeps from the thoracolumbar fascia toward its humeral insertion at the floor of the intertubercular sulcus. Superior to the fold lies the axillary fossa (armpit), while medially the contour blends into the posterior thoracic wall over the ribs and serratus anterior slips. Laterally, the fold wraps onto the proximal arm, where the posterior axillary line and overlying skin creases help orient surface landmarks. A clean surface landmark. Clinically, the posterior axillary fold matters because it is a reliable external reference for the posterior margin of the axilla during lymph node examination and for planning incisions that avoid excessive tension across the axillary skin. Weakness, tearing, or iatrogenic denervation of the latissimus dorsi (thoracodorsal nerve, C6 to C8) can flatten this fold, a finding that also appears after axillary dissection or when the thoracodorsal pedicle is harvested for a latissimus dorsi myocutaneous flap in breast or chest wall reconstruction. The contour also changes in adhesive capsulitis and chronic shoulder dysfunction, where guarded posture alters scapulothoracic position and makes the fold appear more medial and superior. Use this posterior view for surface anatomy teaching in gross anatomy and physical diagnosis courses, and for atlases or patient education materials that explain axillary landmarks without opening the region.
