Description
Centered on the anterior groin of an adult woman, the inguinal ligament spans obliquely from the anterior superior iliac spine to the pubic tubercle, forming the inferior border of the anterior abdominal wall and the superior margin of the femoral triangle. Superior to the ligament, the superficial inguinal ring opens in the external oblique aponeurosis, with the inguinal canal coursing inferomedially toward the pubis and transmitting the round ligament of the uterus. Deep to the canal, the inferior epigastric vessels ascend medial to the deep inguinal ring, while the femoral artery and vein lie immediately inferior to the ligament, lateral to the femoral canal and its lymphatic contents. Orientation matters here because hernia anatomy is defined by relationships, not labels. Indirect inguinal hernias pass lateral to the inferior epigastric vessels through the deep ring and may track alongside the round ligament toward the labium majus, whereas direct inguinal hernias protrude medial to those vessels through Hesselbach’s triangle. Femoral hernias, more common in women, emerge below the inguinal ligament through the femoral canal and can strangulate against the lacunar ligament, a detail that drives operative planning for open and laparoscopic hernia repair. Small structure, big consequences. The ilioinguinal and genital branch of the genitofemoral nerve are also at stake during dissection around the superficial ring and canal, where injury can leave persistent postoperative groin pain or labial dysesthesia. Teaching applications fit gross anatomy of the abdominal wall, pelvic and perineal anatomy, and surgical clerkship modules on groin hernias, femoral triangle exposure, and lymphatic drainage to superficial inguinal nodes. Medical publishers often pair this anterior female inguinal region plate with operative diagrams (Lichtenstein, TEP, TAPP) and ultrasound correlation for suspected femoral hernia or round ligament varicosities in pregnancy.
