Description
Oriented in anterior anatomical position, the right muscular triangle of the neck is defined medially by the cervical midline from the hyoid bone toward the suprasternal (jugular) notch, laterally by the anterior border of the sternocleidomastoid, and superolaterally by the superior belly of the omohyoid. Within these boundaries, the infrahyoid (strap) muscles form layered planes, with sternohyoid lying superficial to sternothyroid, thyrohyoid continuing superiorly to the hyoid, and the omohyoid crossing obliquely toward the intermediate tendon. Deep to the strap muscles, the laryngeal cartilages and upper trachea occupy the midline, and the thyroid gland sits anterolateral to the trachea, its isthmus typically spanning the second to fourth tracheal rings. Surgical teaching benefits from this specific triangle because it is the working corridor for midline neck access, where surgeons split or retract the strap muscles to reach the thyroid, larynx, and proximal trachea while respecting the ansa cervicalis motor supply that runs on the anterior surface of the carotid sheath and sends branches into the infrahyoids. Tracheostomy planning hinges on these relationships, since the sternohyoid and sternothyroid must be separated and the thyroid isthmus may need division or retraction, all while staying away from the carotid sheath laterally and anticipating variant thyroid vessels in the pretracheal fascia.
